Infant Baptism Requirements and Instructions

“Canon 849 – Baptism, the gateway to the sacraments and necessary for salvation by actual reception or at least by desire, is validly conferred only by a washing of true water with the proper form of words.  Through baptism men and women are freed from sin, are reborn as children of God, and configured to Christ by an indelible character; are incorporated into the Church”

Dear Parents,
Congratulations on choosing the sacrament of baptism for your child.  You are your child’s first teacher in the faith.  During the baptism rites you will be asked to renew your baptismal vows and affirm your Catholic faith, to better witness to and nurture your child’s spiritual journey.  Below are requirements and instructions for baptism.
 

1. Completed Baptism Packet
   1. Application Form
   2. Sponsor Forms (per sponsor)
   3. $100.00 Stole Fee (Per Child)
   4. Copy each child’s Birth Certificate
   5. Parish Registration Card (if not registered in any parish)
2. Additional Documentation (depending on the circumstance)
   1. Letter of permission from home parish (non-parishioners)
   2. Sponsors proof of confirmation/letter of good standing from pastor (per sponsor)
   3. Letter or certificate of completion of a baptismal prep course (for those unable to complete the parish’s baptismal prep class or if they have completed the course within the past year).
3. Sponsor Requirements
   1. They should ordinarily be 16 years of age or older.
   2. They must be Catholics who have received Baptism, Confirmation, and first Holy Communion.
   3. They must lead a life in harmony with the Catholic faith.
   4. If they are married, they must be married in the Catholic Church.
   5. Parents of the person to being initiated may not serve as godparents/sponsors.

The packet must be completed in full and submitted to the office for review at least 2 weeks prior to the requested date of baptism.  If the packet and all supporting documents are not received by the deadline the baptism will be postponed until all documents have been received.  Once received and after reviewing all the documents the Parish office will schedule an interview date with the pastor or his associates.  Once complete the Baptism Coordinator will contact each family to schedule the baptism prep class. Baptisms are normally scheduled for the 4^th Sunday’s after the 11:00am mass at St. Michael Church.  Special request for SPP or any other dates are at the discretion of the pastor or priest/deacon conferring the sacrament.
